Entry Fee $20.00
Any non-profit, school or church organization ( please circle one ) may enter free of charge, however, you must submit an entry form.
Line-up and Parade details will be mailed following receipt of completed entry form and entry fee. ENTRIES MUST BE RECEIVED NO LATER THAN AUGUST 31, 2009.

1. Entrant agrees to abide by all reasonable directives issued by authorized parade personnel.
2. Entrant agrees that it's parade entry will be in substantial compliance with the descriptive information provided on this form.
3. Entrant assumes full responsibility for the safety of it's parade entry and all participants involved in such parade entry. It is expressely agreed by the parade entrant that Jesse James Festival, Inc. shall bear no responsibility for the safety of well being of such entries or their participants.
